Area VI Forms Library


Here are the necessary forms and lists which you will find useful (or mandatory) in participating in competitions, clinics and other USEA sponsored activities.  They will "Pop up" on a separate screen in .pdf format and are printable or available for copying or saving so that you can use them on another computer if necessary. All forms should be filled out completely and mailed to the proper address which is usually found on the form itself.

USEA Forms

United States Eventing Association Membership Application You will have to pay an extra fee to compete at the lower levels if you are not a member.  Membership brings you a rule book, the quarterly Omnibus schedule of events, and a subscription to Eventing magazine, with it's lists of current rule changes, articles helpful to eventers, and event results.

Horse Life Registration Allows tracking of horse's performance for both competition eligibility and awards purposes.  Horses above a certain level must be registered with USEA

2008 USEA CIC Entry Form

2008 USEA Horse Trials Entry Form Remember to fill all entries out completely and including all pertinent statistics or they may be rejected.  Send them in as close to opening date as you can, as many events are over-subscribed. Late entries are not accepted, and at most events entries are accepted on a first-come basis.

Stabling Form - USEA  It's especially important to send these with entries, especially if you want to end up in the same area as your friends or coach/trainer.

Emergency Medical Release Form  Another MUST.

Event Evaluation Form  This is also available at the entry desk at events.  It's good to fill these out so that events, courses and officials can keep on getting better and better. Organizers frequently refer to them to assess necessary changes. You can also fill this out online

USEA Rider Awards Application Helps keep the tally straight on awards for which you may be eligible.... especially important for breed, age-specific, and Pony Club or Young Rider awards.

USEA Sponsored Educational Activity Application Requirements  From a landowners or organizer's point of view, having USEA sponsored events has some distinct advantages.  Check out the list of requirements.
